t\u00edpico del GPAA ha sido el siguiente: farmacoterapia, trabeculoplastia l\u00e1ser con arg\u00f3n (ALT), CGM y cirug\u00eda de incisi\u00f3n, con las t\u00e9cnicas ciclodestructivas de alto riesgo reservadas a una fase terminal con mal pron\u00f3stico para la visi\u00f3n.\r\n\r\nEl tratamiento l\u00e1ser subumbral presenta un perfil de riesgo sustancialmente mejor y la SLT est\u00e1 ahora indicada como tratamiento de primera l\u00ednea para un manejo eficaz del GPAA y la hipertensi\u00f3n ocular.\r\n\r\nEl algoritmo de tratamiento actual sit\u00faa la SLT en primer lugar y la t\u00e9cnica SubCyclo sustituye a las t\u00e9cnicas de ciclodestrucci\u00f3n, pues resulta adecuada en fases mucho m\u00e1s tempranas del proceso de la Sin embargo, dado su perfil de riesgo asociado, la ALT no se considera una opci\u00f3n de tratamiento de primera l\u00ednea.\r\n\r\nEn torno a la misma \u00e9poca, el Doctor en Medicina Mark A. Latina, de la Universidad Tufts de Boston, desarroll\u00f3 el concepto de trabeculoplastia l\u00e1ser selectiva (SLT).\r\n\r\nLa SLT induce una respuesta biol\u00f3gica de reducci\u00f3n de la PIO con una eficacia equivalente, pero sin el da\u00f1o t\u00e9rmico y los cambios estructurales irreversibles en la malla trabecular propios de la ALT.\r\n\r\nEl tratamiento l\u00e1ser subumbral presenta un perfil de riesgo sustancialmente mejor y la SLT est\u00e1 ahora indicada como tratamiento de primera l\u00ednea para un manejo eficaz del GPAA y la hipertensi\u00f3n ocular<sup>2<\/sup>. De hecho, la ciclocrioterapia fue una de las primeras t\u00e9cnicas utilizadas para destruir parcialmente el proceso ciliar, prueba de ello, la ciclofotocoagulaci\u00f3n transescleral l\u00e1ser, TSCPC, con un l\u00e1ser de rub\u00ed, descrita por Beckman et al., en 1972.\r\n\r\nLas mejoras progresivas realizadas en la t\u00e9cnica y la tecnolog\u00eda, a lo largo del tiempo, han dado lugar a opciones de l\u00e1ser de diodo, que resultan m\u00e1s seguras e igual de eficaces. Se ha demostrado que la ciclofotocoagulaci\u00f3n (ThermoCyclo) utilizando 810\u00a0nm, es una forma sencilla y no invasiva de tratar el glaucoma avanzado resistente al tratamiento<sup>1<\/sup>.
